At Growlife Medical we are proud to have established a dedicated post-ADF Discharge Clinic, to assist ADF veterans with organising their healthcare after discharge.
Our experienced General Practitioners, many with direct Defence Force experience, combine an understanding of medicine, Medicare, DVA and the unique nature of ADF service in order to help you get your healthcare back on track after discharge, especially in Medical Discharge.
The goals of the clinic are to:
1. Convert your JEHDI/DeHS medical record into a civilian equivalent (Best Practice) and upload this to MyHealthRecord.
2. Using the DVA Veteran Health Assessment Tool and our own experience with ADF veterans, establish a Medical Care Plan that works with your health, employment and functionality to produce the best possible outcome for you.
This Clinic is 100% bulk billed, through support from Federal Government funding available through Medicare to enable this activity - ADF Post-Discharge Health Assessment.
